award forms

How can I get the award forms to include the center and the secretary info?
Also, I cannot get the program to print state assn. awards.
I have tried everything I can think of.

In order to provide the appropriate product support, we created specific forums with the name of the product in the name of the forum.

This forum, as it states in the description, "This is not a forum for product support. This forum is a peer driven forum dedicated to the discussion between secretaries or centers in terms of managing/running leagues or tournaments, rules discussions and similar concepts. This includes discussions, such as average or handicap rules, league formats, league retention, bowler retention or recruiting, etc."
